New Patient Registration Process

We are pleased and excited that you have decided to entrust West Atlanta Internal Medicine with your healthcare needs. Our streamlined process should make it fast and easy to get established:

1. First, please call the office at (770) 942-6903 to schedule an appointment and have your medical insurance card available. Otherwise, you may fax us an enlarged, clear, front and back copy of your insurance card to (770) 942-6908.  This will expedite your initial visit tremendously.

2. Our office policy is to schedule patients as quickly as possible. Generally, we have either same day or next day appointments available for urgent matters.

3. Next, download, print and complete the New Patient Registration Form, which provides us with pertinent information about you and the insurance policy holder.

4. Please read the Georgia Patients' Bill of Rights and HIPAA privacy notice, then download, print and sign the Acknowledgement Form . This indicates that you have read the documents and understand your rights as a patient. If you have any questions about the form, please contact our office and we will be happy to answer your questions.

5. Download, print and complete the Authorization to Release Medical Records Form and send it to your current physician(s). This will authorize your former doctor(s) to forward your medical records to West Atlanta Internal Medicine for Dr. Johnson's review.

6. Please bring with you either a list of your medications with the dosages or the actual pill bottles for the physician to review them thoroughly and carefully.

7. When you arrive for your first appointment with West Atlanta Internal Medicine, please bring your completed forms and medical insurance card. It is best to arrive about 20 minutes early so we can review your forms prior to your appointment.

8. Having followed all of the above-mentioned steps should significantly reduce your wait time; however, please understand that emergencies can and will occur in the office. So, if your schedule does not allow you to wait, please make the staff aware and we will gladly reschedule your appointment. Our goal is to please all of our patients by being courteous and answering all of their questions without feeling rushed.
